Job summary

Oak Hill is hiring in Torrington, CT for a part-time Direct Support role. You’ll provide daily living support, health monitoring, and assist with medications while helping individuals maintain independence. The schedule includes evenings and weekends, with paid training and opportunities for advancement.

Work involves community transport, meal support, and documentation in the EHR. You’ll join a compassionate team focused on safety, inclusion, and quality of life for people with disabilities.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene needs based on individual preferences and support plans.
• Monitor and document health-related information, including vital signs when required; administer medications as directed.
• Transport individuals to appointments, activities, and events using agency vehicles.
• Prepare meals and snacks according to menus and dietary requirements; provide mealtime support.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities, health information, progress notes, incidents, and medication administration in the EHR.
• Follow emergency procedures and safety protocols; apply behavioral support strategies and give First Aid/CPR when required.
• Communicate effectively with team, individuals served, families, and stakeholders; participate in meetings and trainings.
