Job summary

MMD Community Care is a New Jersey DDD approved statewide provider delivering Community Support, Respite, and Individual Support to adults with disabilities. DSPs support clients in home and community settings, under agency direction, promoting independence and inclusion.

Qualifications

• High school diploma or equivalent.
• Valid New Jersey Driver’s License (driving record not to exceed 5 points). If transporting consumers, proof of automobile insurance is needed.
• Must be 21 years or older.
• Must clear state/federal background checks, the Central Registry and CARI database.
• CPR/FA certification.
• Proficient in English (read, write & comprehend).
• Complete all training for the position within established timelines.

Responsibilities

• Assist consumer in development of community living skills and daily activities.
• Plan and participate in recreational activities chosen by the participant.
• Administer medication consistently and timely, following procedures if applicable.
• Develop and maintain relationships with family and friends.
• Foster independence and maximize involvement in daily living skills.
