Job summary

Mentor Community Services, part of Sevita, seeks a Direct Support Professional/Caregiver to provide person-centered support in home and community settings in Hyde Park, Mattapan, and Dedham, MA. The role offers evening and weekend shifts with compensation of $20.25 per hour plus 0.62 cents per mile.

This is a non-exempt, part-time or full-time hourly position. You will help with daily living activities, medication administration when needed, documentation, and community integration while

Qualifications

• High school diploma or equivalent required.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Maintains healthy and professional relationships with individuals, friends, families, guardians and case managers; greets family members and other visitors; implements Sevita Customer Service Standards.
• Reports any instance of alleged abuse or neglect according to internal and external standards; reports medical, behavioral and other incidents in accordance with Sevita policy and external requirements.
