Job summary

Qualifications

• Ability to pass required background checks and complete training provided by the company.

Responsibilities

• Provide direct, hands-on support to individuals in their homes and communities.
• Assist with daily living activities, including personal care and meal prep.
• Support participation in community activities, appointments, and outings.
• Follow individualized care and behavior support plans as directed.
• Document services accurately per NJ DDD requirements.
• Communicate with families and care teams about needs and progress.
• Maintain a safe, respectful, person-centered environment at all times.
