Job summary

Aveanna Education Services is seeking a Direct Support Professional to provide compassionate, person-centered support to individuals with intellectual and developmental disabilities in home and community settings. You will work one-on-one to promote safety, health, and independence while honoring each person’s preferences and goals.

You will implement ISP plans, assist with daily living activities, document services, and collaborate with a multidisciplinary team to ensure outcomes are met.

Qualifications

• High school diploma or equivalent.
• Active driver’s license and auto insurance.
• Willingness to accompany those you serve into the community.
• Eligibility for State Police Criminal Record Check and Child Abuse History Clearance.
• FBI clearance if PA residency requirements are not met.
• Must be 18 years or older.
• Flexible hours and overtime availability.

Responsibilities

• Provide one-on-one support to individuals in home and community settings.
• Help individuals achieve ISP outcomes and promote wellbeing.
• Complete documentation for EVV and daily service notes.
• Collaborate with interdisciplinary team for ISP development and maintenance.
• Report changes in client condition to manager promptly.
• Adhere to confidentiality and professional boundaries.
• Assist individuals in developing life skills and independence.
