Job summary

BrightStar Care Fort Wayne is seeking Direct Support Professionals (DSPs) to provide compassionate, one-on-one care in clients’ homes in Bluffton, IN. The role offers consistent hours, supportive leadership, and meaningful work, with opportunities across full-time and part-time shifts.

You will assist with activities of daily living, mobility, meals, and companionship, while maintaining HIPAA privacy and client dignity. CPR certification and reliable transportation are required.

Qualifications

• High school diploma or GED is required.
• At least one year of professional DSP/ caregiving/ home care experience.
• Current CPR certification is required.
• Physical exam and TB test within the past 12 months are required.
• Valid driver's license, auto insurance, and reliable transportation are required.
• Ability to lift up to 50 pounds and perform physical duties as required.
• Able to pass background check and drug screen.
• Strong communication and documentation skills.
• HIPAA privacy compliance required.

Responsibilities

• Assist with ADLs (bathing, grooming, dressing, personal hygiene).
• Support daily routines, encouraging independence.
• Provide companionship and emotional support.
• Assist with meals, light housekeeping, errands, and activities.
• Support mobility, transfers, and safety needs.
• Perform ROM and mobility exercises per care plan.
• Follow each client’s individualized care plan.
• Complete timely and accurate care documentation.
• Observe and report changes to the nurse case manager.
• Protect client dignity, safety, privacy, and personal choices.
