Pathway 2 Hearts LLC is seeking Direct Support Staff to assist residents in an Adult Family Home setting. The role focuses on maintaining well-being, independence, and quality of life through daily living support, hygiene, mobility, and safety.

Responsibilities include medication administration, health monitoring, documentation, and collaborative communication with healthcare professionals and families. CPR/First Aid may be required; a valid driver’s license is often needed.

Qualifications

• High School Diploma or equivalent.
• Previous experience in a similar role or relevant training preferred.
• CPR and First Aid certification may be required.
• Ability to show empathy, patience, and compassion.
• Driver's License and reliable transportation required.

Responsabilités

• Assist residents with activities of daily living, including bathing, grooming, dressing, and toileting.
• Provide support for mobility and transfers as needed.
• Administer medications as prescribed and monitor health.
• Observe and report changes in residents' health or behavior to healthcare professionals.
• Engage residents in meaningful activities to promote socialization and recreation.
• Assist with meal preparation and ensure dietary needs are met.
• Maintain accurate records of care, progress, and incidents.
• Communicate effectively with residents, families, and staff.
• Follow safety protocols and participate in emergency procedures.
