Job summary

Qualifications

• High School Diploma or equivalent is required.
• Must be at least 21 years of age.
• Must have a valid driver's license.

Responsibilities

• Provide one-on-one or group instruction/assistance to patients daily.
• Maintain data collection sheets and report to supervisor timely.
• Assist with assessment of clients' strengths and needs as necessary.
• Encourage independence and participation in activities.
• Demonstrate a positive, empathetic and professional attitude toward customers.
