Job summary

Trinity Health PACE is seeking a Day Center Aide to support participants with personal care, mobility, and daily living activities. You will work with an interdisciplinary team to monitor health status and ensure safety in a patient-centered, home-like environment.

The role requires an active CNA in the state where PACE operates, at least one year of relevant experience, and a willingness to train if needed. Employment includes a strong benefits package and opportunities for growth.

Qualifications

• Must be an active Certified Nursing Assistant (CNA) in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience with frail or elderly population or training from PACE on hire.
• Experience in long-term care or geriatric programs preferred.

Responsibilities

• Participate in interdisciplinary team assessments and monitor participants.
• Provide personal care assistance to enable activities of daily living (toileting, showers, eating, grooming).
• Assist in maintaining physical activity, exercise, and walking.
• Provide nursing care per policies of the PACE Organization.
• Ensure participant safety and report any changes or hazards.
• Offer behavioral supervision and emotional reassurance to participants.
• Assist with meals, recreation, and housekeeping tasks.
• Identify signs of health or behavioral changes and report to staff.
• Adhere to infection control, PPE, blood-borne pathogens, hazardous materials handling, and fire safety standards.
• Escort participants to medical and other appointments as needed.
• Assist transportation staff with loading/unloading participants.
