Job summary

Qualifications

• Education: High School Diploma or equivalent.
• Six months of experience in human services, direct care, or care coordination preferred.
• Strong communication and interpersonal skills required.
• Patient, compassionate, reliable, and responsible.
• Valid driver’s license and access to a registered vehicle with proof of insurance.

Responsibilities

• Provide compassionate, person-centered care to participants.
• Support with personal hygiene and daily living activities.
• Assist participants with budgeting, exercise, and nutrition goals.
• Help participants engage in community activities and social events.
• Collaborate with supervisors and care teams to ensure participant wellbeing.
