Job summary

Riveon Mental Health and Recovery seeks a dedicated professional to provide case management for clients with severe mental illness across office, home, and community settings.

Essential duties include crisis intervention, coordinating with providers, and developing evidence-based interventions. Candidates should have a psychology/social work background, plus strong documentation skills and computer literacy.

Qualifications

• Bachelor's degree in Psychology, Social Work, Sociology or related field.
• Associate's degree (or near completion) with 3+ years of experience with individuals with severe mental illness.
• Experience in mental health preferred.
• Basic computer skills including Windows and Microsoft Office.
• Willingness to travel with valid Ohio driver’s license and car insurance.

Responsibilities

• Provide/Coordinate crisis intervention as needed with individuals, families, and groups.
• Collaborate with social service providers, medical providers, families, landlords, and staff on treatment plans.
• Develop solution-focused interventions and emotional/behavioral management from evidence-based treatments.
• Identify strategies or treatment options related to each client’s mental illness.
• Help clients restore social skills for work, school, home and daily living tasks.
• Assist clients in linking to resources, coping strategies, and support networks.
• Ensure timely follow-up care according to medical necessity.
• Participate in Quality and Utilization Management functions and record reviews.
• Maintain organization to meet deadlines and complete documentation.
• Provide education and support to families and service providers while protecting confidentiality.
• Complete all required documentation and outcomes within set timelines.
• Maintain productivity and attend required staff meetings; adapt to change.
