Responsibilities

• Provide direct patient care under RN or LPN supervision.
• Assist with activities of daily living (bathing, dressing, grooming, feeding).
• Record vitals such as temperature, pulse, and blood pressure.
• Assist with resident mobility and transfers using proper techniques.
• Maintain a clean, safe, and supportive environment.
• Offer emotional support and companionship to residents and families.
• Document care and report changes in condition to nursing staff.
• Support care teams in implementing individualized care plans.
