Job summary

Graceville Rehab is seeking a dedicated Certified Nursing Assistant (CNA) to join our faith-based care team in Graceville, FL. You will provide direct patient care under RN/LPN supervision, assist with activities of daily living, monitor vital signs, support mobility, and help maintain a safe, compassionate environment.

Ideal candidates have a high school diploma, complete a state-approved CNA program, hold an active CNA certification, and demonstrate strong communication skills.

Qualifications

• High school diploma or equivalent.
• Completion of a state-approved CNA training program.
• Active CNA certification in good standing.
• Strong interpersonal and communication skills.
• Physical ability to lift, move, and support residents safely.

Responsibilities

• Provide direct patient care under the supervision of a Registered Nurse or Licensed Practical Nurse.
• Assist with activities of daily living, including bathing, dressing, grooming, and feeding.
• Monitor and record vital signs such as temperature, pulse, and blood pressure.
• Assist with resident mobility, turning, transfers, and ambulation using proper techniques.
• Maintain a clean, safe, and supportive environment.
• Offer emotional support and companionship to residents and their families.
• Assist in the documentation of care and report changes in condition to nursing staff.
• Support the care team in developing and implementing individualized care plans.
