Job summary

TEEMA Solutions Group is seeking a Certified Nursing Assistant (CNA) to provide compassionate, patient-centered care within a supportive nursing team. The CNA assists with activities of daily living, monitors patient conditions, and helps maintain a clean, safe therapy environment per facility policies.

Qualifications include a current CNA certification in the state of practice, with a preferred BLS certification and recent LTC/SNF experience.

Qualifications

• Current CNA certification in the state of practice.
• Current BLS certification preferred or required per facility policy.
• Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, hospital, or acute care preferred.

Responsibilities

• Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, feeding, and mobility.
• Obtain and document vital signs, weights, intake/output, and other assigned observations.
• Assist with patient transfers, positioning, ambulation, and range-of-motion exercises.
• Observe and promptly report changes in patient condition to the licensed nurse.
• Maintain accurate and timely documentation of patient care.
• Ensure a clean, safe, and comfortable environment while following infection prevention and safety standards.
• Collaborate with nurses and the interdisciplinary healthcare team to support quality patient care.
• Maintain patient confidentiality and comply with HIPAA and facility policies.
• Perform other duties as assigned.

Education

High School Diploma or equivalent
