Qualifications

• Must be over 18 years of age.
• High school diploma or GED.
• Must pass the TABE test.
• Must be able to intervene physically in behavioral and health related crisis with individuals.
• Must complete habilitation aide certification.
• Nurse’s aide experience or some personal care experience preferred.

Responsibilities

• Intervenes on behalf of individuals served and reports to the appropriate agency and/or supervisor.
• Maintains reliable attendance and provides coverage for shifts.
• Monitors individuals’ health needs and contacts nurses as needed.
• Records data and maintains accurate, up-to-date individual records.
• Assists with daily living skills and follows individual plans and lesson plans.
• Ensures safety and follows disaster procedures and uses equipment properly.
