Job summary

Memory Care Center at Emerald in Claremore, OK is seeking a dedicated CNA to join our skilled nursing facility team. You will provide high-quality care to residents, supporting comfort, safety, and well-being.

The role involves assisting with activities of daily living, mobility, vital signs, documentation, and collaboration with nurses and physicians. Candidates should hold an Oklahoma CNA certification and be able to lift and assist residents.

Qualifications

• Experience in a skilled nursing facility or similar healthcare setting preferred.
• Ability to lift and move residents, and perform physical tasks.
• Current Oklahoma CNA certification is required.

Responsibilities

• Resident Care: assist with activities of daily living including bathing, dressing, grooming, and personal hygiene.
• Mobility Assistance: transfers, positioning, safe ambulation.
• Vital Signs Monitoring: take and record vital signs.
• Documentation: accurately document care and changes in residents’ conditions in EHRs.
• Communication: report changes to nursing staff or supervisor promptly.
• Supportive Tasks: assist with feeding, meal prep, and companionship.
• Infection Control: follow protocols and maintain a clean environment.
• Team Collaboration: work with nurses and physicians to ensure cohesive care.
