Responsibilities

• Provides direct care to residents under supervision.
• Assists with activities of daily living and mobility.
• Ensures resident safety and comfort as per care plans.
• Coordinates dining and meal assistance, documents intake.
• Assists with weight and skin condition monitoring and reporting.
• Records progress and communicates changes to nurse staff.
• Performs other duties as assigned.
