Berlin Health & Rehabilitation Center in Barre, Vermont is seeking a Certified Nursing Assistant to provide direct patient care and support residents’ daily living activities. You will work with licensed staff to ensure compassionate, high-quality care across ADLs, meals, mobility, and documentation.

Responsibilities include monitoring vital signs, assisting with admissions and transfers, and maintaining a safe, respectful environment.

Qualifications

• Vermont CNA certification required before start date.
• High school diploma preferred.
• New grads welcome; CNA or caregiver experience preferred.

Responsibilities

• Assist residents with ADLs, including bathing, grooming, oral care, and incontinence care.
• Assist with meals and feeding as needed.
• Support mobility, transfers, ambulation, and wheelchair assistance.
• Turn and reposition residents to maintain comfort.
• Assist with range-of-motion exercises.
• Respond promptly to call lights and resident requests.
• Measure and record vital signs, weight, intake, and output.
• Assist with admissions, discharges, and transfers.
• Observe and report changes in residents' condition to the nursing staff.
• Maintain a clean, neat, and safe resident environment.
• Complete daily documentation accurately and on time.
• Perform other duties as assigned.
• Provide respectful, compassionate, resident-centered care at all times.
