Grace Healthcare Services is seeking a Certified Home Health Aide (CHHA) to provide compassionate, in-home care across Burlington, Gloucester and Camden counties. Starting wage is $20 per hour, based on experience, with full-time daytime shifts from Monday to Friday and occasional weekends.

You will assist with bathing, grooming, meal prep, mobility, and daily living activities, document care with CellTrak, and coordinate with RN Case Managers to monitor changes in patients’ conditions, all

Qualifications

• Ability to read, write, and communicate fluently in English.
• Physical ability to lift, push, and pull up to 50 lbs.
• Reliable transportation and willingness to travel within Burlington County.

Responsabilités

• Provide personal care including bathing, grooming, dressing, skincare, and mobility.
• Prepare simple, nutritious meals per care plans.
• Maintain a safe, clean home environment.
• Document care accurately using CellTrak.
• Collaborate with RN Case Managers to monitor patient conditions.
