21st Century Home Health Services (21HHS) is hiring a Certified Home Health Aide for Alameda County (Dublin, Livermore, Pleasanton and surrounding cities). You will work under an RN Case Manager to carry out the Plan of Care and support patients in their homes.

The role emphasizes compassionate, high-quality personal care, collaboration with an interdisciplinary team, and adherence to safety protocols. Attractive benefits and a path for growth are offered.

Qualifications

• Valid CHHA CA Certification.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience in home health care preferred.
• Experience with high-acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Provide personal care including baths, hygiene, and changing bed linen.
• Assist with dressing and grooming.
• Provide skin care to prevent breakdown.
• Assist with toileting and feeding as needed.
• Report changes to the assigned nurse.
• Meet safety needs and use equipment properly.
• Keep living areas clean and orderly.
• Complete required computer literacy assessment prior to scheduling interview.
