Job summary

Swan Hospice is seeking a CHHA with hospice experience to join our team. We provide care in skilled nursing facilities, assisted living facilities, and private homes with a compassionate approach to patients and families.

Ideal candidates will hold an NJ HHA license, understand hospice philosophy, and have at least one year of hospice experience. Reliable transportation is preferred to support patient visits.

Qualifications

• Minimum of one year of hospice experience.
• HHA license in NJ is required.
• Understands the hospice philosophy.

Responsibilities

• Assists with prescribed exercise and ROM activities as taught by the Hospice Nurse.
• Provides assistance in toileting, positioning, transfers, and turning of bed-bound patients.
• Assists with ADLs including personal hygiene, grooming, mouth, skin care, and nails.
• Assists with medication reminders in compliance with regulations.
• Performs light housekeeping in the resident's unit and handles patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respiration as supervised by the Hospice Nurse.
• Reports changes in patient condition to the Hospice Nurse and documents communications.
