Responsibilities

• Provide personal care tasks (bathing, grooming, dressing, skincare, mobility).
• Prepare simple, nutritious meals per care plans.
• Maintain a safe, clean home environment for patients.
• Document care using CellTrak.
• Collaborate with RN Case Managers to report changes in patient conditions.
