Job summary

Distinctive Living in Warren, NJ, is seeking a CHHA/CNA for a part-time, per diem role at our Mira Vie Warren community. The candidate will provide direct care to residents and support individualized care plans while maintaining a safe, clean environment.

The role emphasizes collaboration with the interdisciplinary team, adherence to state and federal regulations, and ongoing professional growth within a caring workplace.

Qualifications

• High School Diploma or equivalent required.
• Six months of caregiving experience.
• CNA or HHA certification required.
• Excellent verbal and written communication skills.
• Experience with Word and Excel preferred.

Responsibilities

• Receives and gives report to management at the beginning and end of each shift and as needed.
• Provides personal care to residents per their individualized plan of care.
• Documents all pertinent information regarding care as assigned.
• Prepares residents for meals and assists to and from the dining room.
• Assists residents with meal setup and eating.
• Assists residents with toilet and incontinence care.
• Keeps residents' apartments neat and clean.
• Performs clinical procedures according to facility standards and scope of practice regulations.
• Performs vital signs, BP, weights and other trained procedures as directed by management.
• Completes prevention and restorative care including PROM, AROM, ambulation, transferring, bathing, grooming, and all other ADL care.
• Maintains a working knowledge of facility infection control procedures.
• Assists in turning, positioning, and repositioning of residents as outlined in the resident's service plan.
• Participates in Interdisciplinary Team Process and attends care plan meetings as assigned.
• Communicates effectively with residents, families, visitors, and health care team.
• Participates in facility's Quality Assurance Program.
• Maintains education development and compliance with State and Federal Regulations.
• Has a basic knowledge of the organization to answer questions.
• Follows facility policies regarding emergency procedures.
• Maintains resident confidentiality and adheres to HIPAA policies.
• Performs other duties as assigned.
