Job summary

BayMark Health Services is seeking a compassionate Certified Substance Use Counselor to support patients through medically assisted treatment and coping strategies, with individual and group sessions to manage crisis and progress.

You will collaborate with doctors, nurses and other counselors to ensure effective counseling and develop treatment and aftercare plans in an outpatient setting; CA certification and background checks required.

Qualifications

• Certified Counselor in the State of California.
• Experience with chemically dependent individuals is a plus.
• Satisfactory drug screen and criminal background check.

Responsibilities

• Provide individual and group counseling services; direct service hours at regularly scheduled intervals; coordinate all services throughout treatment
• Completes Bio-psychosocial assessment.
• Facilitates, establishes and reviews with each patient the initial treatment plan
• Develop aftercare plans and discharge plans
• Ensure efficient and effective delivery of counseling services to all patients
• Other related duties as determined by supervisor
