You will collaborate with residents, healthcare providers, and other stakeholders to develop individualized care plans and advocate for resources.

The role requires compassion, strong communication, EMR proficiency, and the ability to work within a multidisciplinary team in a fast-paced setting.

Qualifications

• Experience in case management or care coordination in healthcare or social services.

Responsibilities
