Job summary

You will lead care conferences, manage admissions/discharges, and supervise licensed staff, while upholding regulatory standards and a resident-first approach in a dynamic long-term care setting.

Qualifications

• Associate degree required.
• RN or LPN license in Washington required.
• Sub-acute care or Skilled Nursing experience preferred.
• Strong communication and customer service skills.
• Highly organized with strong documentation abilities.
• Ability to collaborate with interdisciplinary teams.
• Proficiency with MS Office and healthcare systems including Answers on Demand, ADP, DocSTAR, FaxCORE, and mobile technology.

Responsibilities

• Oversee care experience from admission to discharge.
• Coordinate admissions, discharges, transfers, and bed management.
• Lead care conferences and family conferences.
• Ensure evidence-based practices and regulatory compliance.
• Collaborate with internal and external stakeholders.
• Provide leadership to licensed staff and NACs.
• Participate in weekly Case Management meetings.
