Responsibilities

• Provide patient-centered care tailored to each client’s needs.
• Assist with activities of daily living, including mobility and grooming.
• Prepare meals and snacks as needed.
• Provide medication reminders and assist with medication management.
• Offer companionship and social activities to support mental well-being.
• Perform light housekeeping and run errands as needed.
• Report changes or concerns to the care coordinator or supervisor.
