Job summary

Wiley Creek Senior Living is seeking a Care Partner to support Assisted Living residents with activities of daily living and to provide companionship that enhances emotional well-being. You will assist with grooming, meals, mobility, and personal care, while maintaining accurate documentation and reporting health changes to the Health Services team.

The role emphasizes compassionate, patient-centered care and collaboration with staff to adapt to resident needs and shifts in conditions.

Qualifications

• Requires a high school diploma or equivalent.
• Experience in caregiving or health care settings preferred.
• Excellent English communication, both written and verbal.

Responsibilities

• Assist residents with activities of daily living (ADLs) as needed.
• Provide companionship to promote emotional well-being.
• Report incidents and changes in health to supervisory staff.
• Assist with dining, grooming, mobility, toileting and basic care per care plan.
• Maintain accurate resident documentation and charts.
