Responsibilities

• Develop comprehensive transition plans integrating clinical, care coordination, academic, family, and classroom supports.
• Provide therapeutic services to identified students to support transition back to general education.
• Lead care coordination components of each student's plan and collaborate with school staff and families.
• Offer individual and small-group clinical support using trauma-informed practices.
• Support academic success through consultation on accommodations/interventions and skill-building.
• Help students build social-emotional, coping, and self-regulation skills.
