Compact Licensed Physical Therapist Position #5 (Care Navigator) - REMOTE

Full-Time, Remote, $38 USD per hour. Location: Remote.

Job Overview

As a Care Navigator, you will support patient success by identifying their needs and challenges, assisting in implementing interventions to guide behavior change, and supporting musculoskeletal health. The role is entrepreneurial and team-focused, requiring a passion for engaging users to achieve health goals.

Responsibilities

• Provide remote monitoring services to support patients as they progress through their home-exercise therapy program.
• Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through the technology platform.
• Provide ongoing feedback to the evaluating clinician on patient progress outside the clinic.
• Maintain necessary records and notes during remote monitoring.
• Assist with improving processes and functionality of the platform.
• Organize and manage a caseload while staying efficient.

Requirements

• Licensed Physical Therapist with a compact state license.
• 2+ years of experience in musculoskeletal health is a plus.
• Graduated from an accredited physical therapy program.
• Tech-savvy; comfortable switching between EHR, messaging, and scheduling systems.
• Experience in behavioral modification; Health Coach Certification is a plus.
• Excellent communication, interpersonal, and presentation skills.
• Strong relationship‑building and listening skills.
• Demonstrated patient focus and genuine passion for user experience.
• Ability to multi‑task and manage various user types.
• Ability to work independently with limited supervision.
• Flexibility and agility to work cross‑functionally in a fast‑paced environment.
