Pride Health is hiring for a Community Wellness Advocate II (Remote) to support our client’s medical facility based in Costa Mesa, CA. This is a 14-week (Temp to Hire) opportunity and a great way to start working with a top-tier healthcare organization!

Responsibilities:

  • Responsible for working on various projects aimed at closing quality care gaps for Managed Care patient populations, including health plan special programs/initiatives.
  • Coordinate delivery of quality services through patient outreach, appointment scheduling, chart abstractions at various clinics, engaging with provider clinics, point of care testing, and coordinating the closure of quality care gaps.
  • Demonstrates high knowledge of departmental processes and procedures, as well as measures from the National Committee for Quality Assurance (NCQA), and the Centers for Medicare & Medicaid Services (CMS), and other recognized agencies.
  • Engage in education with clinic staff to provide feedback as needed.
  • Act as an internal trainer and provide guidance to all team members.
  • Collaborate with the Quality Improvement team to review data, prioritize outreach, and contribute to department and organizational goals.
  • Demonstrates and practices a workstyle that reflects the organization's mission, vision, and values.
  • Participate in the continuous Quality Improvement process, maintaining knowledge of departmental standard operating procedures, including the use of specialized instrumentation, quality control requirements, and preventive maintenance.
  • Duties can include coordinating care for patients, sending messages, requesting referral information, reviewing records, etc.

Required Skills & Experience:

  • Three (3) to five (5) years in a healthcare system environment.
  • Experience or familiarity with front-office or scheduling procedures in healthcare system environment.
  • Basic keyboarding skills at 35 WPM.
  • Basic knowledge of Microsoft Office Suite, including Outlook, Word, and Excel.

Preferred Skills & Experience:

  • Qualified bilingual or multilingual candidates.
  • Epic experience is preferred, but not mandatory.

Preferred Certifications & Licensure:

  • Certification as a Medical Assistant by an accredited program.
  • Licensed Vocational Nurse by an accredited program.
  • BLS certification.

Job Title: Community Wellness Advocate II

Location: Costa Mesa, CA. (Role will be primarily remote but the candidate needs to be local as there may be some onsite needs. Travel will not be paid for, so the candidate needs to reside locally.)

Pay Range: Local- $20 to $26/hr (*Offered pay rate will be based on education, experience, and healthcare credentials.)
