Responsibilities

• Conduct outreach in Louisville in high-risk areas to prevent violence.
• Establish relationships with high-risk individuals and provide immediate support.
• Identify and intervene in potential conflicts to prevent violence; facilitate mediation.
• Connect high-risk individuals with Outreach Case Coordinators and Community Health Workers.
