The Louisiana Department of Health seeks a dedicated professional to coordinate transitions from nursing facilities to community living, linking participants with essential resources and partners.

You will implement waiver programs, track outcomes, and support staff through training and ongoing guidance to promote safe, independent living for older adults and individuals with disabilities.

Qualifications

• Experience developing, managing, or evaluating health or social service programs.
• Experience in public health, public relations, social services, health services, or health regulation.
• Graduation-based substitutions allowed for higher education hours.

Responsabilités

• Coordinate and monitor participant transitions from nursing facilities to community living.
• Implement operational protocols for waivers and long-term care programs.
• Coordinate with nursing facilities, regional offices, state agencies, and partners to facilitate waiver offers.
• Maintain and update transition records, spreadsheets, and program data for reporting.
• Connect participants with housing assistance and community resources.
• Monitor program performance, benchmarks, and indicators to identify trends.
• Provide technical assistance and training to agency staff and partners.
