Job summary

New Horizons Behavioral Health (CAI program) seeks an individual for the DD Community Access role serving Southern Counties. The position offers up to 30 hours per month at $15.90 per hour and requires a high school diploma or GED with at least 2 years’ experience supporting individuals with DD, mental illness, or addictions.

Applicants must have an insured vehicle, clean driving record, and a valid driver’s license.

Qualifications

• High School diploma or GED required.
• At least two years of experience working with individuals with developmental disabilities, mental illness, and/or addictive diseases, or 2 years of providing direct care.
• Must have an insured vehicle, clean driving record, and a valid driver's license.

Responsibilities

• Transport and engage individuals to and from community outings and events of their choice.
• Electronically document the day’s activities and the reaction of the individual in their electronic medical record.
• Communicate with immediate supervisor regularly.
• Attend annual ISP meetings.
• Other duties may be assigned.
