Job Summary: We are seeking a Community Support Worker [CSP] | Recovery Support Navigator to provide case management and community-based services tailored to individual client needs, collaborating with providers, family, and community resources to improve functioning and access to services.

Responsibilities:

• Assess clients' non-medical/mental health needs.
• Develop, implement, and monitor service plans.
• Handle authorization requests and re-authorizations.
• Communicate effectively with clients and stakeholders.
• Coordinate with internal departments and attend meetings.
• Ensure documentation is complete and accurate.
• Conduct consultations and bill as needed.
• Participate in relevant training.

Minimum Qualifications:

• Bachelor’s degree in psychology or related field.
• Understanding of population-specific and privacy protocols (HIPAA).
• At least 1 year of relevant experience.
• Bilingual preferred (Spanish/English).
