Job summary

Hillcrest Children Center is seeking a Community Support Worker to promote mental health and address behavioral health needs among children, adolescents, adults, and families. You will conduct needs assessments, manage 25-40 cases, and maintain meticulous records in compliance with policy.

Responsibilities

• Provide culturally and linguistically appropriate post-placement services and comprehensive case management for unaccompanied children (UC).
• Complete individualized needs assessments and develop case plans, considering culture, language, and special circumstances.
• Manage a caseload of 25-40 cases.
• Maintain high standards of record-keeping per USCRI and ORR policies.
• Provide information, education, referrals, outreach, advocacy, and support to ensure each child receives necessary services.
• Continuously assess placement safety and appropriateness using child welfare principles via calls, interviews, and home visits.
• Build a network of local pro bono legal resources and service providers for UC and sponsors, and track progress.
• Coordinate and maintain communication with key stakeholders.
• Participate in regular training and sessions with supervisor and headquarters staff.
• Perform other duties as assigned by the supervisor.

Job description

The Community Support Worker works with individuals to promote optimum mental health and address behavioral health issues of service population. This position works directly with children, adolescents, adults or family groups to provide individualized treatment in dealing with issues associated with mental and emotional health. The Community Support Worker is responsible for the facilitation of the client’s transition to independence; endorsement and application of community resources and the provision of information and referral services including mental health resources.

Duties and Responsibilities:

• Provide culturally and linguistically appropriate post-placement services and comprehensive case management for unaccompanied children (UC).
• Complete individualized needs assessments and develop case plans, considering culture, language, and special circumstances.
• Manage a caseload of 25-40 cases.
• Maintain high standards of record-keeping per USCRI and ORR policies.
• Provide information, education, referrals, outreach, advocacy, and support to ensure each child receives necessary services.

Qualifications:
