Job summary

Epic Nursing Services is seeking a Community Case Worker to support AFC and GAFC programs. You will conduct case management, perform home visits, and coordinate services to help adults 22+ remain safe and independent in their homes.

The role requires strong communication, organizational skills, knowledge of community resources, and a valid driver's license. Experience in care coordination is preferred.
