Job summary

Carolina Outreach seeks a full-time CST Professional Team Lead to deliver community-based therapy and case management for adults with serious mental health challenges. The role centers on empowering clients to function and achieve goals in the community.

Responsibilities include assessments, individual/family therapy, person-centered plans, crisis response, and coordination with care teams. Licensing in NC is required and EMR experience is preferred.

Qualifications

  • Master’s degree from an accredited university in Counseling or a related field.
  • Valid licensure to provide mental/behavioral healthcare in North Carolina (LCSW, LCSWA, LCMHC, LCMHCA, LMFT or Licensed Psychologist).
  • Minimum of two years of experience with mental/behavioral health, including writing person-centered plans and group therapy.
  • Valid driver’s license, auto insurance and reliable transportation.
  • EMR experience preferred.

Responsibilities

  • Provide individual, family, and/or group therapy sessions.
  • Maintain accurate and timely clinical documentation.
  • Collaborate with care teams and participate in supervision as applicable.
  • Ensure compliance with clinical and regulatory standards and complete progress notes.

This role focuses on empowering clients to enhance their ability to function and achieve their goals within the community.Therapeutic and Clinical ServicesConduct comprehensive initial assessments and reassessments to evaluate client needs and strengths.Provide individual and family therapy using evidence-based practices to support the mental health and well-being of clients.Develop and implement person-centered plans to guide service delivery.Monitor and manage symptoms through psycho-education and therapeutic interventions.Facilitate client referrals to higher levels of care, such as ACT services, when appropriate.Case Management and AdvocacyAssist clients in accessing and coordinating benefits, resources, and services in the community.Act as a liaison between clients, families, and community resources to ensure service continuity and effectiveness.Educate clients and families on available community resources, benefits, and service options.Provide ongoing support for rehabilitative and independent living skills development.Community-Based SupportDeliver services in off-site, community-based settings to foster client engagement and success.Respond to client crises promptly and effectively, providing support and guidance as needed.Documentation and ComplianceMaintain accurate, timely, and compliant documentation, including service notes, treatment plans, and progress reports.Monitor and evaluate the quality of services provided to ensure compliance with agency standards and client needs.This description outlines the general scope and level of work for this role.
