Job summary

Serene Health, part of Optima Medical Management Group, is seeking a Community Support Supervisor in Sacramento to ensure efficient operation of Community Support services for Health Plan members. You will supervise CS staff, generate daily/weekly reports, coordinate with health plans for referrals and housing deposits, and ensure proper documentation.

The role requires a high school diploma; Bachelor's preferred, 4+ years related experience, and a valid CA driver’s license.

Qualifications

• High school diploma or GED required; Bachelor’s preferred.
• Bachelor's degree in social work or healthcare administration, or at least 4 years of related work experience.
• Previous supervisory experience in community support services or healthcare.
• Valid driver's license, reliable daily vehicle, and CA auto insurance.

Responsibilities

• Generate daily and weekly reports for member progress.
• Maintain and update the Master Log daily.
• Review RingCentral Analytics, productivity logs, and member notes.
• Oversee staff notes and authorization processes in Alliance.
• Coordinate with health plans for referrals and authorizations, ensuring compliance.
• Ensure timely completion of housing deposit processes with landlords.

Job Summary:

As a Community Support Supervisor, you will play a pivotal role in ensuring the efficient operation of Community Support services to our Health Plan members. You will be responsible for providing supervision, managing daily and weekly reports, coordinating with health plans for referrals and authorizations, overseeing staff assessments, and ensuring housing deposit processes are completed accurately and with proper documentation. Your role will involve effective communication with various partners, including health plans, landlords and members.

Supervisory Responsibilities:

• Provide leadership, guidance, and support to CS staff.

Duties/Responsibilities:

• Generate and analyze daily and weekly reports for member progress.
• Maintainand update the Master Log daily.
• ReviewRing Central Analytics, productivity logs, and member notes.
• Oversee staff notes and authorization processes in Alliance.
• Collaboratewith health plans for referrals and authorizations, ensuring compliance.
• Stayinformed on health plan policies and procedures.
• Coordinatecompletion of Health Plan Housing Goods Form.
• Ensurequality and consistency in staff assessments.
• Monitorassessment processes for regulatory adherence.
• Coordinatetimelycompletion of housing deposit processes, verifying accuracy with landlords.
• Communicateeffectively with stakeholders,maintainingaccuratedocumentationand promptlyaddressinginquiries and requests.
• Performsother duties as assigned.

Education and Experience:

• High school diploma or GEDrequired. Bachelor’s degree preferred.
• Bachelor's degree in social work or healthcare administration, or at least 4 years of related work experience.
• Previousexperience in a supervisory role, preferably in community support services or healthcare.
• A valid Driver's License, reliable dailyvehicleand active CA auto insurance arerequired.

Pay range

$29 - $32.50 USD
