Job summary

Legacy Treatment Services in New Jersey is seeking a full-time behavioral health professional to support individualized habilitation plans and IMR-based interventions. The role entails coordinating both mental health and physical health services while maintaining an active schedule and meeting productivity requirements.

Responsibilities include crisis intervention as needed, and delivering services aimed at restoring functioning and community integration, with opportunities for training, tuition

Qualifications

• Bachelor’s Degree in a helping profession (social work, counseling, psychiatric rehabilitation, or psychology).
• If degree is in another field, minimum two years of relevant community-based behavioral health experience.
• Certified Psychiatric Rehabilitation Practitioner (CPRP) may substitute for one year of experience.

Job description

Contributes to the development, implementation, monitoring, and updating of Consumers’ individualized habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements. Other duties as assigned by management staff. Productivity requirement of 60% face to face contact weekly.
