My Life My Home in Illinois is seeking a Direct Support Professional to passionately support residents in their home, in CILA settings of 4-6 people, with evening shift responsibilities and an hourly starting pay.

This role involves personal care, meals, hygiene, and documenting care plans. Must be 21+, valid driver's license, HS diploma, and pass background checks; a reading quiz is required.

Qualifications

• Must be 21+ years old with a valid driver's license and fewer than 3 moving violations.
• HS diploma or GED is required with proof provided.
• Background check and physical are required, including DCFS/CANTS abuse checks.
• Must pass reading and comprehension quiz.

Responsibilities

• Provide in-home care and support to residents in CILA settings.
• Assist with meals, personal care, hygiene, and daily activities.
• Follow each resident's care plan and document daily progress.

The individuals who reside at the CILAs are provided daily supports from a team of dedicated Direct Support Professionals, this may include personal care, cooking and assistance with meals, personal hygiene and grooming, cleaning inside their home, supporting each care plan and daily documentation. • PT Evening shift; mainly 3 pm-11 pm some Saturdays may be a 11am-7pm shift

Starting base pay $19.05-21.05 an hour

Additional $1.00 increase to base pay after completing medication pass certification.

REQUIREMENTS: Must be 21 years or older and have a valid driver's license with less than 3 driving offenses. A High School diploma or GED is required and must provide proof. A criminal background check and physical is required to meet requirements of the position. The background check includes the DCFS/CANTS check for abuse/neglect with children. You must also be able to complete and meet requirement of a reading and comprehension quiz.
