Qualifications

• At least 21 years of age and valid driver's license with reliable transportation.
• CPR and First Aid certification required (can be provided on hire).
• Must pass background checks and pre-employment drug screening.
• Completion of CSA training within six months of hire per agency timelines.

Responsibilities

• Maintain an active caseload of CCSS clients and provide person-centered, strengths-based services.
• Conduct functional and needs-based assessments; update service plans every 90 days.
• Provide skill-building interventions in home and community settings.
• Ensure timely, accurate documentation consistent with Medicaid and agency standards.
• Facilitate team meetings and interdisciplinary collaboration.
• Complete CSA and CCSS trainings; support onboarding and staff development.
