R&C Consulting Group, Inc. in North Carolina seeks a Community Support Team Associate Professional to provide community-based behavioral health support, help clients build daily living skills, access resources, and stay engaged in treatment under clinical direction.

Collaborate with QPs, families, and providers, document progress accurately, uphold HIPAA and NC Medicaid requirements, and contribute to person-centered recovery outcomes within office and community settings.

Qualifications

• Meets North Carolina Associate Professional (AP) classification requirements for the CST service.
• Education, training, and experience meet NC DHHS and NC Medicaid requirements.
• Demonstrated interest or experience in behavioral health, human services, or a related field.
• Ability to communicate effectively with clients, families, coworkers, and community partners.
• Ability to complete accurate documentation and follow established procedures.
• Valid driver’s license and reliable transportation when required for community-based duties.

Responsabilités

• Provide assigned community-based support services consistent with the client’s approved service plan and the employee’s qualifications.
• Help clients identify personal strengths, needs, recovery goals, and barriers to treatment.
• Assist clients with accessing housing resources, transportation, employment services, educational opportunities, and other community supports.
• Encourage participation in treatment, recovery activities, and appropriate community programs.
• Promote independence, social functioning, and positive community integration.
• Carry out interventions consistent with approved service plans and supervisory guidance.
• Observe and report progress, barriers, and changes in client needs.
• Report changes to appropriate supervisor and participate in team meetings.
