Responsibilities

• Provide community-based support per service plan.
• Help clients identify strengths, recovery goals, and barriers to treatment.
• Assist with housing, transportation, employment resources and other supports.
• Document progress and maintain records in accordance with standards.
• Collaborate with QPs and CST team; report safety concerns.
