Job summary

Ozarks Healthcare in West Plains, Missouri is seeking a Clinical Professional to join our team full-time during daytime hours (8 a.m. – 5 p.m.).

The role centers on supporting individuals with mental health needs by providing direct, indirect, and community-based services to an assigned caseload. Responsibilities include documenting activities in the patient records, coordinating care with supervisors, promoting independence and recovery, and helping clients access financial, housing, and crisis

Qualifications

• Bachelor’s degree in psychology, sociology, social work or other mental health field preferred.

Responsibilities

• Provides daily, direct, and indirect services to assigned caseload; meeting with the person served where they are at in order to assist the individual with daily living skills, educating them regarding diagnosis and treatment, accessing services and resources as needed including financial assistance, medical assistance, housing assistance, crisis services, and meeting their immediate needs of food, clothing, and shelter.
• Client care will be monitored by properly documenting all related activities on the appropriate forms in a timely manner and by keeping the supervisor informed of all clinical consults, critical incidents, and crisis.
• Maintains a well-organized work environment including tidy work space, proper time management skills, a well-managed calendar, and appropriate chart compliance.
• Monitors and encourages attendance of all appointments of persons served including but not limited to therapy, groups, medication services, and PSR.
• Promotes recovery, independence, self-care, and responsibility by helping develop natural supports within the community.

Community Support Specialists help individuals develop skills, access resources, and learn to self-manage illnesses in order to be successful in life, work, education, and social settings. Community Support Specialists teach, model, and practice skills with persons served in order to increase self-sufficiency and independence. They provide daily, direct, and indirect services to an assigned caseload; meeting with the person served where they are at in order to assist the individual with daily living skills, educating them regarding diagnosis and treatment, accessing services and resources as needed including financial assistance, medical assistance, housing assistance, crisis services, and meeting their immediate needs of food, clothing, and shelter.
