Job description

Compensation: $25.54/hour; $26.81/hour (Master's)

The Community Support Specialist (CSS) provides comprehensive, community-based support services to adults with behavioral health and substance use disorders. The CSS collaborates with clients in various settings to help achieve personal recovery and rehabilitation goals.

ESSENTIAL FUNCTIONS:

Clinical Service Provision

  • Offer personalized services using a motivational, stage-matched approach, focusing on recovery/resiliency goals.
  • Assist clients in identifying their strengths, needs, and preferences to develop Wellness Plans.
  • Provide holistic services, including coaching on the Eight Dimensions of Wellness, covering physical and mental health.
  • Support clients in accomplishing Wellness Plan goals, reviewing and updating plans as needed.
  • Help clients access public services like housing, financial aid, and emergency assistance.
  • Maintain contact with hospitalized clients, participate in discharge planning, and coordinate aftercare with providers.
  • Educate and build skills in daily living, employment, symptom management, communication, budgeting, and residence maintenance, aiming for independence or referral to long-term supports.
  • Assist clients in developing personal support networks, including family, legal guardians, and recovery communities.
  • Help clients access mental health services, including accompanying them to appointments.
  • Follow up on appointments, forms, and receipts.
  • Complete assessments and Wellness Plans as directed.
  • Meet service expectations and complete documentation timely.

Integrated Health Team

  • Communicate with team members regarding client wellness, medication adherence, side effects, and medication efficacy.
  • Incorporate input from health team providers into Wellness Plans.
  • Make referrals to additional services as needed.
  • Serve as a liaison to others involved in the client’s treatment, such as family, employers, and courts.

Miscellaneous

  • Provide transportation support as needed; driving is an essential function.
  • Perform other duties as assigned.

Minimum Qualifications:

  • High School/GED required.
  • Degree in a human service-related field preferred (Associate’s, Bachelor’s, or Master’s).

Work Experience:

Must meet at least one of the following:

  • Qualified addiction professional;
  • Bachelor’s degree in a human services field from an accredited U.S. institution;
  • Any four-year degree or combination of education and experience;
  • Four years of qualifying experience;
  • Associate of Applied Science in Behavioral Health Support (designation by the department).

Qualifying experience must include providing services to individuals with mental illness, substance use disorders, or developmental disabilities. A valid driver’s license and appropriate automobile coverage are required.
