As part of this mission, we are seeking a dedicated and experienced Community Support - Lead Care Program Manager to oversee our community-based programs and lead a team in delivering high-quality, whole-person care.

Job Summary:
The Community Support - Lead Care Program Manager plays a critical role in implementing and managing community-based support services for Medi-Cal beneficiaries. This position is responsible for developing comprehensive strategies, overseeing service delivery, and ensuring alignment with Medi-Cal Transformation goals. The Program Manager will work with various stakeholders to address social determinants of health and support community-based initiatives.

Key Responsibilities:

  • Team Management:Lead and manage a team of Community Support Lead Care Managers to provide high-quality, whole-person care that addresses physical, behavioral, and social needs.
  • Strategic Development:Create and implement a comprehensive strategy for community-based support programs, focusing on whole-person care, care coordination, and value-based services in line with Medi-Cal Transformation goals.
  • Stakeholder Collaboration:Collaborate with Medi-Cal beneficiaries, healthcare providers, community organizations, and government agencies to identify and address the unique needs of the Medi-Cal population.
  • Service Design & Delivery:Oversee the design and implementation of evidence-based, culturally responsive support services that address social determinants of health, including housing, food security, transportation, and employment.
  • Professional Development:Provide leadership, guidance, and professional development opportunities to the Community Supports team to ensure consistent, high-quality service delivery.
  • Partnership Development:Develop and maintain partnerships with community-based organizations, social service agencies, and key stakeholders to enhance service coordination and maximize resources.
  • Program Evaluation:Monitor and evaluate program effectiveness using data-driven approaches to measure outcomes, identify improvement opportunities, and demonstrate value within the Medi-Cal Transformation framework.
  • Funding & Budget Management:Secure and manage funding for community support programs, including grant writing, budget oversight, and financial reporting.
  • Compliance:Ensure all programs comply with federal, state, and local regulations and adhere to Medi-Cal Transformation guidelines and requirements.

Qualifications:

  • Education:Bachelor’s degree in Social Work, Public Health, Health Administration, or a related field; a Master’s degree is preferred.
  • Experience:Minimum of 3 years in managing community support services, preferably within the Medi-Cal or Medicaid system.
  • Knowledge:Strong understanding of Medi-Cal Transformation initiatives, goals, strategies, and best practices in community-based support services.
  • Leadership Skills:Proven leadership abilities with excellent communication and interpersonal skills, capable of building relationships with diverse stakeholders.
  • Budget & Resource Management:Experience in managing budgets and resources in a value-based payment environment.
  • Technical Skills:Proficiency in using technology and data management systems to support service delivery, reporting, and decision-making.
  • Licenses:Valid driver’s license and ability to travel within the community as required.
