Job summary

Responsibilities

• Confers with participants, parents, social workers to develop Individual Plans.
• Maintains supports and services for goals within budget allotted.
• Reviews plans quarterly for progress or barriers.
• Creates and maintains confidential files of participants.
• Develops and maintains community resources for plan implementation.
• Coordinates learning materials aligned with Individual Plans.
