Job summary

Community Counseling Center seeks a Community Support Caseworker to deliver community-based support and case management to adults and children. You will work in client homes and residential centers, managing a caseload of 20–25 clients with weekly meetings.

Responsibilities include training clients in daily living skills, monitoring treatment goals, coordinating with the treatment team, and referring to community resources to improve functioning and independence.

Qualifications

• Bachelor's degree or four years of qualifying experience or a four-year combination.
• Qualifying experience includes service delivery to individuals with mental illness, substance abuse disorders, or developmental disabilities.
• Must hold or obtain MO Class E Driver's License by start date.

Responsibilities

• Deliver community support services for enrolled clients within CPRC.
• Work in community settings such as client homes or residential centers.
• Manage a caseload of 20–25 clients with weekly meetings.
• Provide training/education to clients in daily living skills.
• Monitor treatment plan goals and maintain contact with the treatment team.
• Refer clients to community resources to improve functioning and support networks.
• Document according to clinical policies and program standards.
