Job summary

North Mississippi State Hospital seeks a qualified professional to provide Community Support Services (CSS). You will obtain and maintain CSS certification, manage case loads, and coordinate services with other agencies while delivering client-centered support.

You will conduct ongoing needs assessments, develop treatment plans, assist with housing, benefits, medications, and advocate for clients, attending meetings and completing documentation daily.

Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field.
• Ability to relate effectively to severely mentally ill individuals.
• Have transportation, hold a valid driver’s license, have automobile insurance, and have a safe driving record.
• Must have, and maintain, a Communicare insurable driving record.

Responsibilities

• Obtain and maintain proper certification to perform CSS and submit applications independently.
• Meet productivity requirements by planning ahead and coordinating with supervisor when deficiencies are anticipated.
• Provide case management and ongoing assessment, develop treatment plans.
• Coordinate case management and services with other agencies.
• Provide supportive counseling to clients and their families as needed.
• Assist CSS clients in making applications for benefits and housing.
• Maintain proper documentation and complete reports as requested.
